Attach attested photocopies of the following documents with the UKJ application:
Matric Certificate. FSC certificate or equivalent
• MBBS degree with detailed marks certificate of all professionals
• Certificates of Effort
• PM&DC Valid Permanent Registration Certificate
• House job certificate with correct dates from concerned Medical Superintendent as well as Professor/Head of Department of concerned hospital (both are required)
• Domicile certificate
• Computerized National Identity Card (CNIC)
Attach experience certificate (if any).
• Three passport size photographs duly attested on front/back (to be pasted on the admission form)
• Foreign qualified candidates are required to submit HEC/PMDC equivalent certificate.
• Application forms are available from the account section of FPGMI. 2000/- Lahore on payment of Rs.
Application with incomplete documents will not be considered for admission.
• During the course of training, if any discrepancy is found in the documents submitted, the admission will be cancelled.
• Applications received after the due date or with incomplete information will not be considered.
• Applicants who are in service and desirous of admission on deputation basis should apply through proper channel.
